ABSTRACT

Objetivo: Identificar o impacto da histerectomia para patologias benignas sobre a sexualidade feminina. Métodos: Revisão de literatura com busca na plataforma PubMed, sendo selecionados 23 artigos em português e inglês publicados entre 2016 e 2021. Resultados: Foi descrita, majoritariamente, melhora na função sexual após histerectomia, semelhante às abordagens totais ou supracervicais e independentemente da via de acesso cirúrgico, apesar de impacto ligeiramente menor com a via laparoscópica. Na laparoscopia, houve melhor desfecho sexual no fechamento da cúpula vaginal, quando comparado ao fechamento via vaginal. Ademais, a ooforectomia concomitante apresentou resultados conflitantes e inconclusivos. Conclusão: A histerectomia afeta positivamente a saúde sexual feminina e aspectos técnicos podem interferir na função sexual, porém os dados são limitados. Devido à importância do tema, necessitam-se de mais estudos com metodologias padronizadas para possibilitar análises mais detalhadas.


Objective: To identify the impact of hysterectomy for benign pathologies on female sexuality. Methods: Literature review with search on PubMed platform, being selected 23 articles in Portuguese and English published between 2016 and 2021. Results: Improvement in sexual function after hysterectomy was mostly described, being similar in total or supracervical approaches and independent of the surgical access route, although it had slightly lower impact when laparoscopic. In the laparoscopic approach, there was better sexual outcome in the vaginal dome closure when compared to vaginal closure. In addition, concomitant oophorectomy showed conflicting and inconclusive results. Conclusion: Hysterectomy positively affects female sexual health and technical aspects may interfere with sexual function, but data are limited. Due to the importance of the theme, more studies with standardized methodologies are needed to enable more detailed analyses.

ABSTRACT

Gestational hypertension and pre-eclampsia are important causes of perinatal morbidity. The objective of the present study was to determine the increase in relative risk for developing hypertensive disorders of pregnancy based on the evaluation of pregnant women between 20 and 25 weeks of gestation, and to correlate the findings at this period with the outcome of pregnancy. We conducted a prospective cohort study, with a convenience sample of 1417 patients evaluated at this gestational age, of which 1306 were contacted at childbirth. We detected an increased relative risk of 2.69 (95%CI: 1.86 to 3.89) associated with pulsatility index of the uterine arteries, a 2.8 increase (95%CI: 1.58 to 5.03) in relative risk attributed to maternal age above 35 years, a 1.68 increase (95%CI: 1.17 to 2.40) attributed to parity greater than or equal to 3, and a 5.35 increase (95%CI: 4.18 to 6.85) attributed to chronic hypertension and obesity, with a progressive increase in relative risk according to the degree of overweight, i.e., grades 1, 2, 3, and morbid obesity (2.58, 3.06, 5.84, and 7.28, respectively).

ABSTRACT

O presente relato descreve uma condição rara de intussuscepção uterina em uma cadela sem raça definida, quatro anos de idade, diagnosticada por meio de celiotomia exploratória e análise anatomopatológica. Foi realizada ovariossalpingo-histerectomia (OSH) como tratamento. Essa patologia geralmente requer manejo cirúrgico porque o diagnóstico inicial pode ser desafiador.(AU)


The present study describes a rare condition of uterine intussusception in a 4 year old crossbred female dog diagnosed by exploratory celiotomy and anatomopathological analysis. As treatment, a ovariohysterectomy (OH) was performed. This pathology usually requires surgical management because the initial diagnosis can be challenging.(AU)

Abstract Purpose: To evaluate the effects of oxidative stress in pregnant rats submitted to acute and chronic stress, relating to alterations in the uterus, placenta and fetus. Methods: Twenty-four female Wistar albino (Rattus norvegicus), were divided into four groups, for induction of oxidative stress the animals were submitted to cold and physical immobilization. Plasma fasting glucose and MDA were determined in all groups and the fetuses and placentas were measured. Results: There were no statistical differences in the levels of malonic dialdehyde (MDA), however the averages of chronic stress group were higher compared to control groups, which could explain the observed adverse effects; there was no correlation between puppies' size, the weight of the placenta and MDA values. Conclusions: Chronic stress causes adverse effects, when compared to control groups; chronic stress group had fetuses, placentas and number of puppies, significantly lower compared to other groups. The rats exposed to chronic stress, also presented a higher frequency of fetal resorption.

ABSTRACT Uterine contractility out of the gestational phase, during the menstrual cycle and the habitual functional variations of the organ, this is one of the responsible mechanisms for reproduction and fertility, due to its direct action in the mechanisms conducting the spermatozoa to the ovule and in the decidual implantation. Pathologies such as uterine leiomyoma, endometriosis, adenomyosis, polycystic ovarian syndrome, as well as the use of intrauterine devices and oral contraceptives, may alter a functionality of uterine contractility. Thus, magnetic resonance imaging with ultrafast sequences provides a dynamic evaluation (cine-MRI) and thus the correlation of uterine contractility quality in patients with current infertility or pathologies.


RESUMO A contratilidade uterina fora da fase gestacional, durante o ciclo menstrual e as habituais variações funcionais do órgão, é um dos mecanismos responsáveis pela reprodução e fertilidade, devido sua ação direta nos mecanismos de condução dos espermatozoides até o óvulo e na implantação decidual. Patologias como leiomioma uterino, endometriose, adenomiose, síndrome dos ovários policísticos, bem como o uso de dispositivos intrauterinos e anticoncepcionais orais, podem alterar a funcionalidade da contratilidade uterina. Desta forma a ressonância magnética com sequências ultra-rápidas proporcionam uma avaliação dinâmica (cine-RM) e assim a correlação da qualidade da contratilidade uterina em pacientes com infertilidade ou patologias vigentes.

ABSTRACT

OBJECTIVES: Doppler ultrasonography can be used to assess neoangiogenesis, a characteristic feature of postmolar gestational trophoblastic neoplasia. However, there is limited information on whether uterine artery Doppler flow velocimetry parameters can predict gestational trophoblastic neoplasia following a complete hydatidiform mole. The purpose of this study was as follows: 1) to compare uterine blood flow before and after complete mole evacuation between women who developed postmolar gestational trophoblastic neoplasia and those who achieved spontaneous remission, 2) to assess the usefulness of uterine Doppler parameters as predictors of postmolar gestational trophoblastic neoplasia and to determine the best parameters and cutoff values for predicting postmolar gestational trophoblastic neoplasia. METHODS: This prospective cohort study included 246 patients with a complete mole who were treated at three different trophoblastic diseases centers between 2013 and 2014. The pulsatility index, resistivity index, and systolic/diastolic ratio were measured by Doppler flow velocimetry before and 4-6 weeks after molar evacuation. Statistical analysis was performed using Wilcoxon’s test, logistic regression, and ROC analysis. RESULTS: No differences in pre- and post-evacuation Doppler measurements were observed in patients who developed postmolar gestational trophoblastic neoplasia. In those with spontaneous remission, the pulsatility index and systolic/diastolic ratio were increased after evacuation. The pre- and post-evacuation pulsatility indices were significantly lower in patients with gestational trophoblastic neoplasia (odds ratio of 13.9-30.5). A pre-evacuation pulsatility index ≤1.38 (77% sensitivity and 82% specificity) and post-evacuation pulsatility index ≤1.77 (79% sensitivity and 86% specificity) were significantly predictive of gestational trophoblastic neoplasia. CONCLUSIONS: Uterine Doppler flow velocimetry measurements, particularly pre- and post-molar evacuation pulsatility indices, can be useful for predicting postmolar gestational trophoblastic neoplasia.

ABSTRACT

Background: Polycystic ovary syndrome [PCOS] is one of the most common hormonal disorders that can lead to irregular menstrual cycles and hyperandrogenism. Reduced levels of progesterone and increased estrogen in these women can perpetually stimulate the endometrial tissue of the uterus. In this study, we assess the effect of PCOS induction by estradiol valerate [EV] in a rat model


Materials and Methods: In this experimental study, adult female Wistar rats that weighed approximately 200 g were divided into control, sham, and experimental groups [n=6 per group]. The experimental group received subcutaneous injections of 2 mg EV for induction of PCOS. We confirmed the presence of PCOS in the experimental group rats. Rats from all groups were subsequently killed, after which their uteri were removed and fixed for histological and cytological analyses. The uterine tissue sections were stained with hematoxylin and eosin [H and E] and iron hematoxylin [iron-H]. We examined epithelium height, thickness of the uterus wall, and frequency of the mitotic cells. The data were assessed at alpha=0.05


Results: Uterine tissue findings from the experimental group showed significant increases in the height of the uterus luminal epithelium, the thickness of the uterus wall, and the frequency of eosinophils in the endometrial stroma. We observed an increased frequency of mitotic cells in the experimental group in both luminal and glandular epithelia of the uterus. An increased rate of the glandular epithelium region was noticeable and significant


Conclusion: Induction of PCOS by EV could change the proliferation rate in the endometrial tissue of the uterus

ABSTRACT

In this study, the effects of high-intensity-focused ultrasound (HIFU) treatment on benign uterine tumor patients were examined. A total of 333 patients diagnosed with fibroids or adenomyosis using diagnostic sonography, treated with HIFU between February 4, 2010 and December 29, 2014 at a hospital in Korea, and followed up for three or six months were selected for this study. Their benign uterine tumor volume was measured, and the effects of HIFU treatment on the volume were analyzed according to age, disease, fertility, and treatment duration. The volume of benign tumors of the uterus changed by age in all age groups after conducting HIFU treatment for 3 and 6 months, respectively. The rate of decrease in individuals' in their twenties was the largest, at 64.9%. When the decreasing volume of benign tumors of the uterus was analyzed by type of disease, the treatment efficacy for adenomyosis was the best, with a decrease of 164.83 cm3 after 6 months. Myoma had the fastest decreasing rate, at 68.5%. When evaluated on the basis of fertility, the volume of benign tumors of the uterus continued to decrease until 6 months after completing all procedures. The volume has continued to decrease for 6 months after all procedures. This study showed that HIFU treatments for uterine fibroid and adenomyosis is an effective non-invasive therapy via reducing the benign uterine tumor volume. Therefore, the HIFU method might replace other conventional treatment methods.

ABSTRACT

A gravidez molar, espectro benigno da doença trofoblástica gestacional, representa uma complicação obstétrica da primeira metade da gestação. De maneira geral, cursa com sintomatologia exuberante: hemorragia, útero aumento para a idade gestacional, cistose ovariana e pré-eclâmpsia precoce; chegando mesmo a graves situações clínicas de hipertireoidismo e insuficiência respiratória.Todavia, devido ao diagnóstico precoce da mola hidatiforme, ainda no primeiro trimestre, mercê da ultrassonografia, houve uma importante redução na ocorrência desses sintomas; contribuindo para que a condução desses casos excepcionais ficasse confinada aos Centros de Referência. É objetivo desse artigo apresentar ao obstetra brasileiro uma revisão das complicações clínicas da gravidez molar, atualizando-o no diagnóstico precoce e tratamento dessas condições clínicas que podem ser potencialmente ameaçadoras à vida da gestante e de seu concepto.(AU)


The molar pregnancy, in the benign gestational trophoblastic disease spectrum, represents an obstetric complication of first half of gestation. In general, runs with exuberant symptoms: bleeding, uterus increase for gestational age, ovarian cistose and early preeclampsia; even serious clinical situations of hyperthyroidism and respiratory failure. However, due to the early diagnosis of hydatidiform mole, still in the first trimester, through ultrasound, there was a significant reduction in the occurrence of these symptoms; contributing so that the conduct of these exceptional cases could be confined to the centers of reference. The purpose of this article is to present to the Brazilian obstetrician a review of clinical complications of molar pregnancy, updating it in the early diagnosis and treatment of clinical conditions that can be potentially threatening to the life of the pregnant woman and her fetus.(AU)

Introdução: a fisiopatologia da pré-eclâmpsia (PE) baseia-se em deficiência no processo de placentação, associada à disfunção endotelial sistêmica materna. É preciso investigar a ocorrência desses fenômenos antes do aparecimento das manifestações clínicas da PE, considerando a disponibilidade de novos métodos propedêuticos de sua predição. Objetivos: comparar o processo de placentação e a função endotelial de gestantes de alto risco e os correlacionar com o desenvolvimento ou não da PE. Pacientes e métodos: 74 gestantes foram submetidas à dilatação fluxomediada (DFM) da artéria braquial e dopplerfluxometria de artérias uterinas para avaliação da função endotelial e do processo de placentação, respectivamente; entre 16 e 20 semanas de gestação e acompanhadas até o puerpério. Resultados: 15 pacientes tiveram a gestação complicada por PE e 59 mantiveram-se normotensas até o puerpério. Pacientes que subsequentemente desenvolveram PE apresentaram, entre 16 e 20 semanas de gestação, elevados valores no índice de pulsatilidade das artérias uterinas (p<0,001), mas a análise da DFM não apresentou diferença em relação às pacientes que se mantiveram normotensas. Conclusão: os dados sugerem que a deficiência no processo de placentação precede cronologicamente as manifestações clínicas de PE, o que não ocorre com a disfunção endotelial.


Introduction: the pathophysiology of pre-eclampsia (PE) is based on a deficiency in the placenta formation process, associated to systemic maternal endothelial dysfunction. The occurrence of these phenomena before the appearance of the clinical manifestations of PE should be investigated, considering that new propaedeutic methods are availablefor prediction. Objective: to compare placenta formation process and endothelial function of high-risk pregnant women and correlate them with the occurrence of PE. Patients and methods: 74 pregnant women were subjected to flow mediated dilation (FMD) of the brachial artery and Doppler flux of uterine arteries to assess the endothelial function and the placenta formation process, respectively, between the 16th and 20th gestation weeks and followed until the puerperal period. Results: 15 patients had their pregnancies complicated by PE while 59 remained normotensive until the puerperium. Patients who subsequently developed PE showed, between gestation weeks 16 and 20, high scores in the pulsatility index for uterine arteries (p<0.001), but the FDM analysis presented no difference in relation to the patients who continued normotensive. Conclusion: data suggest that deficiencies in the placenta formation process chronologically precedes the clinical manifestation of PE, which does not occur with endothelial dysfunction.

FUNDAMENTO: A fisiopatologia da Pré-Eclampsia (PE) é caracterizada por deficiência no processo de placentação, disfunção endotelial sistêmica e hiperfluxo do Sistema Nervoso Central (SNC). Do ponto de vista clínico, seria interessante determinar a ocorrência desses fenômenos antes do aparecimento das manifestações clínicas da doença, levantando a possibilidade de novos métodos de predição da PE. OBJETIVO: Comparar o processo de placentação, a função endotelial e o hiperfluxo do SNC em gestantes de alto risco para desenvolvimento de PE que posteriormente desenvolveram ou não a síndrome. MÉTODOS: Um total de 74 gestantes foi submetido ao exame de Dilatação Fluxo-Mediada (DFM) da artéria braquial, dopplerfluxometria de artérias uterinas e oftálmica para avaliação da função endotelial, processo de placentação e hiperfluxo central, respectivamente. Os exames foram realizados entre 24 e 28 semanas de gestação e as pacientes foram acompanhadas até o puerpério para coleta de dados. RESULTADOS: Quinze pacientes tiveram a gestação complicada pela PE e 59 se mantiveram normotensas até o puerpério. Pacientes que subsequentemente desenvolveram PE apresentaram entre 24 e 28 semanas de gestação, maiores valores no índice de pulsatilidade das artérias uterinas e menores valores de DFM (p < 0,001 e p = 0,001, respectivamente). Entretanto, não houve diferença nos valores obtidos no índice de resistência da artéria oftálmica (p = 0,08). CONCLUSÃO: Os dados obtidos sugerem que a deficiência no processo de placentação e a disfunção endotelial precedem cronologicamente as manifestações clínicas da PE, o que não ocorre com o hiperfluxo do SNC.


BACKGROUND: The physiopathology of Preeclampsia (PE) is characterized by a deficiency in the process of placentation, systemic endothelial dysfunction and Central Nervous System (CNS) hyperflow. From a clinical point of view, it would be interesting to determine the occurrence of these phenomena before the onset of clinical manifestations of the disease, raising the possibility of new methods for predicting PE. OBJECTIVE:Compare the process of placentation, endothelial function and CNS hyperflow in pregnant women at high risk for the development of PE who subsequently developed or not the syndrome. METHODS: A total of 74 pregnant women underwent the Flow-Mediated Dilation (FMD) of the brachial artery, Doppler study of uterine and ophthalmic arteries for the assessment of endothelial function, process of placentation and central hyperflow, respectively. The examinations were performed between 24 and 28 weeks of gestation and were followed until the postpartum period for data collection. RESULTS: Fifteen patients had PE and 59 remained normotensive until the puerperium. Patients who subsequently developed PE had between 24 and 28 weeks of gestation, higher pulsatility index of uterine arteries and lower values of FMD (p < 0.001 and p = 0.001, respectively). However, there was no difference in the values obtained in the resistive index in the ophthalmic artery (p = 0.08). CONCLUSION: The data obtained suggest that the deficiency in the process of placentation and endothelial dysfunction chronologically precede the clinical manifestations of PE, which does not occur with CNS hyperflow.

ABSTRACT

Transvaginal ultrasound is the gold standard for the evaluation of the structural gynecological pathology both benign and malignant, with optimal performance. Currently the functional gynecological pathology is, usually not considered in the routine ultrasound study. This leads to a sub diagnosis of frequent pathologies, for example metrorrhagia or amenorrhea. It is feasible, after discarding the organic possibilities, and relating sonographic markers specific to the clinical status, to achieve an orientation about the origin of the dysfunction of the hypothalamic-pituitary-ovarian-uterus axis.


La ecografía transvaginal es el gold standard para la evaluación de la patología ginecológica estructural tanto benigna como maligna con óptimo rendimiento. Actualmente, la patología ginecológica funcional habitualmente no forma parte del estudio ultrasonográfico de rutina lo que lleva a un subdiagnóstico de patologías frecuentes como por ejemplo metrorragias o amenorreas. Es factible, descartando los cuadros orgánicos y relacionando marcadores ecográficos específicos con el cuadro clínico, lograr una orientación sobre el origen de la disfunción del eje hipotálamo-hipófisis-ovario-útero.

RESUMO OBJETIVO: Avaliar o impacto da embolização arterial de miomas (EAM) sobre o volume uterino (VU), na função ovariana. MÉTODOS: Trinta pacientes com leiomioma se submeteram à EAM. Foram realizados exames de USPTV e FSH antes e três meses após a EAM. Foram analisados o VU em cm³, o diâmetro do mioma dominante (DMD) em cm e o FSH em UI/mL, expressos por média desvio padrão (DP) e submetidos a análise estatística pelo teste não paramétrico de Mann-Whitney. RESULTADOS: Foram incluidos na análise 29 casos. A média do VU pré-EAM foi 402,4 165,9 cm³, DMD pré-EAM 5,9 2,1 cm. O VU pós-EAM foi 258,9 118,6 cm³, DMD pós-EAM foi 4,6 1,8 cm. A média da dosagem de FSH pré-EAM foi 4,9 3,5 UI/mL e pós-EAM foi 5,5 4,7 UI/mL com p=0,5. Houve redução de 35 por cento do VU, de 22 por cento no DMD e a EAM não alterou significativamente os valores de FSH após três meses. CONCLUSÃO: O procedimento diminui significativamente o VU e DMD e, não há aumento significativo dos níveis séricos de FSH, não havendo, portanto, alterações na função ovariana.


PURPOSE: To evaluate the impact of uterine artery embolization (UAE) on uterine volume (UV), greater myoma diameter (GMD) and ovarian function three months after the procedure, by transvaginal pelvic ultrasonography (TVPUS) and by the determination of follicle-stimulating hormone (FSH). METHODS: Thirty patients with leiomyomas were submitted to UAE. TVPUS and FSH determination were performed before and three months after UAE. UV was determined in cm³, GMD in cm and FSH in IU/mL. Data are reported as as mean standard deviation (SD) and were analyzed statistically by the nonparametric Mann-Whitney test. RESULTS: Twenty-nine patients were analyzed. Before UAE, mean UV was 402.4 165.9 cm³ and GMD was 5.9 2.1 cm. After UAE, mean UV was 258.9 118.6 cm³ and GMD was 4.6 1.8 cm. Mean FSH concentration was 4.9 3.5 IU/mL before UAE and 5.5 4.7 IU/mL after UAE, with p=0.5. There was a 35 percent reduction of UV and a 22 percent reduction of GMD, with no changes in FSH values after three months. CONCLUSION: The procedure significantly reduced UV and GMD but did not cause a significant increase in FSH levels, thus causing no changes in ovarian function.

ABSTRACT

Relacionaram-se as características da adenomiose com as fases do ciclo estral em 61 peças de úteros de bovinos colhidas em matadouros. A adenomiose foi classificada em superficial e profunda. A fase do ciclo estral foi estimada pela morfologia, pela coloração e pela vascularização do corpo lúteo e presença ou não de folículos ovarianos maiores que 8mm. Os animais que estavam em anestro (n=11) apresentaram a menor ocorrência de adenomiose (8,2 por cento), e os que estavam na fase lútea média (n=21), a maior (31,0 por cento). Nas fases lútea inicial (n=13) e folicular (n=16) as ocorrências foram semelhantes, 18,0 e 22,9 por cento, respectivamente. A maior porcentagem de adenomiose profunda ocorreu nas fases lútea inicial e média, 45,0 e 47,4 por cento, respectivamente, e durante o anestro e a fase folicular foram de 20,0 e 14,3 por cento, respectivamente. Os resultados sugerem que a fase do ciclo estral influencia na ocorrência de adenomiose e no grau de infiltração miometrial das glândulas endometriais


The relationship of the adenomyosis characteristics and the phases of the estrus cycle in 61 cows bovine's uteruses collected in slaughterhouses was studied. The adenomyoses were classified as superficial and deep. The morphology, staining and vascularization of the corpus luteum and the presence or not of larger ovarian follicles than 8mm helped to estimate tthe estrus cycle. The cows in anestrus (n=11) showed the least occurrence of adenomyosis (8.2 percent) and the animals in the medium luteal phase (n=21) the largest one(31,0 percent). In the initial luteal phase (n=13) and the follicular phase (n=16) the occurrences of adenomyosis were similar and equals to 18.0 and 22.9 percent, respectively. The largests percentage of deep adenomyosis were found in the initial and in the medium luteal phases, 45.0 and 47.4 percent, respectively, and during the anestrus and the follicular phase they were 20.0 and 14.3 percent, respectively. The data suggest that the cycle phase influences in adenomyosis occurrence and in the degree of miometrial infiltration of the endometrial glands

ABSTRACT

As malformações uterinas são achados pouco comuns na clínica ginecológica. As estatísticas nesta área são muito falhas. Corrobora, ainda, a falta de uniformização na terminologia empregada e as dificuldades diagnósticas. A partir da década de oitenta, a ultra-sonografia tornou-se um procedimento indispensável à prática toco-ginecológica, contribuindo e modificando conceitos e procedimentos dentro dessa especialidade. O advento dos transdutores endocavitários, a análise com Doppler colorido de amplitude e espectral, assim como a melhoria crescente da qualidade de imagem contribuíram para isso. Nos últimos dez anos muito se tem pesquisado, publicado e discutido sobre o papel da ultra-sonografia tridimensional. Os autores fazem uma revisão do tema e ressaltam a importância dessa metodologia como modalidade diagnóstica.


Uterine malformations are unusual findings in the clinical practice of gynecology. Statistics regarding this abnormality are flawed, and there is a lack of standardization of the pertinent terminology, besides difficulty in the diagnosis. Starting in the 1980's, ultrasound became a procedure indispensable to the clinical practice in the field of tocogynecology, modifying concepts and procedures, and bringing an important contribution to this specialty. The advent of endocavitary transducers, amplitude and spectral color Doppler, as well as the increasing improvement in the imaging quality, has contributed to this progress. Over the last ten years, much has been studied, published and discussed about the role of the three-dimensional ultrasonography. The authors review the topic and emphasize the importance of this method as a diagnostic modality.

ABSTRACT

Dill [Anethum graveolens] from Umbelliferae is used traditionally to treat convulsions and increasing milk production. Also, in Darab [Fars Province] and Yazd dill fruit is used traditionally to prevent abortion. Its antimicrobial, antihyperlipidaemic, anti-hypercholesteroaemic effects and reducing gastric acid secretion have been reported. The aim of the present study was to investigate the effects of dill fruit hydroalcoholic [DFHE] extract on virgin rat uterus contractions induced by KCI [60mM] and oxytocin [10 mU/ml]. Macerated method was used for extract preparation. Virgin Sprague Dawley female rats were pretreated with an injection of estradiol valerate [5mg/kg, s.c.] 24 hours prior to experiment. Rats were killed and uteri were dissected and mounted in an isolated organ bath containing De Jalon solutions [29°C] bubbled by oxygen. Contractile responses were recorded isometrically under 1g initial tension. The DFHE [0.5, 1, 2, 4 and 8 mg/ml] relaxed both KC1- and oxytocin-induced contractions dosedependently [p<0.0001]. However, the spasmolytic effect of extract on oxytocin-induced contractions was more potent. In Ca[2+] -free, rich K+ [120mM] De Jalon solution, cumulative adding of calcium [0.15, 0.3, 0.6, 1.2 and 2.4 mM], increased contractions dose dependently [p<0.0001]. DFHE [4 mg/ml] shifted this dose-response curve to the right [p<0.05]. In Ca[2+] free solution, oxytocin induced strong contractions if calcium [0.3 mM] was added to the bath. However, DFHE [8 mg/ml] reduced calcium-induced contraction [p<0.001]. The spasmolytic effect of DFHE was reversible. The spasmolytic effect of DFHE [4 mg/ml] was affected neither by propranolol [1micro M] nor by L-NAME [100 micro M]. These results suggest that the relaxatory effect of DFHE on KCI and oxytocin-induced uterus contractions is due, at least in part, to blockade voltage dependent calcium channels and partly by disturbing in releasing calcium from intracellular pool indirectly. In addition, the beta-adrenoceptors and NO are not involved in this inhibitory effect of DFHE. Our results may also support the traditionally usage of dill fruit for preventing abortion

ABSTRACT

La medicina basada en la evidencia ha demostrado mediante una serie de estudios, la eficacia clínica de algunos fármacos, como el caso del misoprostol análogo de la prostaglandina E1 utilizado en obstetricia para lograr maduración cervical en inducción de trabajo de parto con cérvix no favorable, con un efecto no deseado en la mayoría de los casos como lo es la hiperestimulación uterina. Se diseñó un estudio clínico prospectivo entre noviembre, 2002 y febrero, 2003 en la Clínica Rafael Uribe Uribe del Instituto de Seguro Social, Cali, donde se incluyeron 29 pacientes con criterio para inducción del trabajo de parto con índice de Bishop no favorable; se les administró 50 mcg de misoprostol por vía vaginal y desarrollaron polisistolia (>5 contracciones en 10 minutos). A estas 29 pacientes se les suministró 20 mg de nifedipina por vía oral en el momento en que apareció la polisistolia y se estableció mediante el registro por monitoría electrónica maternofetal durante 30 minutos la disminución en el número, la duración de las contracciones y la “actividad uterina”
